Job Description

Minimum Qualifications
Bachelors or Associates Degree in nursing, or closely related field, or Masters in Rehabilitation Counseling.
Minimum of 1 year experience on-site case management with injured worker population in a public or private setting
One of the following credentials is required:
Certified Rehabilitation Counselor (CRC)
Certified Disability Management Specialist (CDMS)
Certified Rehabilitation Registered Nurse (CRRN)
Certified Vocational Evaluator (CVE)
Certified Occupational Health Nurse (COHN)
Must have valid drivers license, automobile insurance and reliable transportation.
Excellent interpersonal, oral and written communication;
Problem
solving and organizational skills;
Computer skills including MS Word and Excel required.

Job Description
Coordinate the delivery of services to a dedicated caseload of 20-25 clients
Conduct initial evaluation process with the injured worker, employer, physician and other interested parties to ascertain the specific issues that prevent a return to work
Develop a comprehensive treatment plan of action with all interested parties to provide an expedient and efficient means to recovery and RTW, including implementation of early return to work or transitional work processes whenever possible
Update changes to treatment plan through on-going assessment and communication with all parties
Provide verbal and written monthly reports
Maintain timely communication with all parties to achieve timely case coordination of services
Maintain comprehensive case management notes and billing tracking activity sheets
Provides reports to Regional Manager as directed
