Job Description

Job Summary

Develop a comprehensive treatment plan of action with all interested parties to provide an expedient and efficient means to recovery and RTW, including implementation of early return to work or transitional work processes whenever possible.

Essential Duties and Responsibilities

All duties and responsibilities are essential job functions and are subject to possible modifications to reasonably accommodate individuals with disabilities.

  • Coordinate the delivery of services to a dedicated caseload of 20-25 clients
  • Conduct initial evaluation process with the injured worker, employer, physician and other interested parties to ascertain the specific issues that prevent a return to work
  • Update changes to treatment plan through on-going assessment and communication with all parties
  • Provide verbal and written reports
  • Maintain timely communication with all parties to achieve timely case coordination of services
  • Maintain comprehensive case management notes and billing tracking activity sheets
  • Provides reports to Regional Manager as directed
  • Monitors and maintains compliance with federal laws and rules, Ohio workers compensation laws and rules, and BWC contract requirements as applicable to areas of responsibility as well as OOD laws and rules as applicable
  • Identify areas for quality improvement and make recommendations to supervisor
  • May be required to perform other job-related tasks other than those specifically presented above





Bachelors or Associates Degree in nursing, or closely related field

One of the following credentials is required or must be obtained within 12 months of hire:

  • Certified Rehabilitation Counselor (CRC)
  • Certified Disability Management Specialist (CDMS)
  • Certified Rehabilitation Registered Nurse (CRRN)
  • Certified Vocational Evaluator (CVE)
  • Certified Occupational Health Nurse (COHN)
  • Certified Case Manager (CCM)


Minimum of 1 year experience case management in a public or private setting

Masters in Rehabilitation Counseling or related field

Two plus years as a field case manager

Working knowledge of Ohio workers compensation and OOD systems

Core Competencies:

Must have valid drivers license, automobile insurance and reliable transportation

Excellent interpersonal, oral and written communication

Problem solving and organizational skills

Able to convey complex medical information in a manner that is easily understood

Proven ability to work independently off-site

Technical Skills:

Computer skills including MS Word and Excel required

Able to navigate electronic files and paperless systems
