Summary of Role:
Care managers assist indivduals to maintain independence, dignity and maximum functionality. Care managers facilitate and coordinate services for clients suffering from the negative impacts of aging, have special needs, become injured/ill or disabled. Care managers also provide assistance to caregivers to more effectively manage physical, emotional and the financial challenges they are faced with.
Responsibilities: - Conduct comprehensive initial assessments to determine medical, physical, nutritional, psychosocial, environmental, legal and financial needs
- Maintain compliance with HIPAA, ERISA and Crawford & Company policies/guidelines
- Identify potential risk factors and provide quality, cost-effective solutions
- Recognize and support cultural diversity when managing overall process
- Integrate services provided by community agencies, home health experts, medical and long term care professionals
- Serve as a client advocate and acts as a liaison for family members and other interested parties
- Provide education, coping skills and information for family members and other involved parties
- Coordinate individualized support services at all levels - local, governmental, home health and long-term care facilities
- Monitor quality of care on an initial and ongoing basis
- Adhere to client specific instructions
- Develop and maintain local community resources and contacts
- Participate in ongoing care management skills development
- Review and analization of records reports and data
- Perfroming job site evaluations/summaries, monthly written reports and recommendation
- Facilitate timely return to work within statutory or union guidelines
- Recommend, facilitate the completion of peer reviews and IMEs
- Review claims files for adjuster and supervisors
- Conduct onsite visits to homes, health care providers and various offices to facilitate return-to work and case resolution.
Required:
- Master's degree in Social Work, bachelor's degree in Nursing or R.N. from accredited diploma program or Certified Rehabilitation Counselor (CRC)
- Experience in gerontology, social work, nursing and/or counseling.
- Knowledge of community and health care resources, and eligibility criteria
- Thorough knowledge of reimbursement resources
- Understating of traditional workers' compensation and disability case management
- Excellent verbal, analystical, and written communication skills
- Strong diplomatic and interpersonal skills
- Possess valid drivers license
